does this person have a cable or DSL modem. Many times there is a public IP address that is given to the Cable or DSL modem. You can then set up the router to forward whichever port your program is using to the specific machine that you are attempting to get a hold of. Then all you need is to find out their public IP address and you can plug that in to VNC to connect to them.

just forward port 3389 to the private ip of the computer of your choice and you will also need to know the ip that there internet provider has assigned to them to type into your computer when you open remote desktop. You can find this ip by simply going to ipchicken.com on one of there systems and it will tell you

I use 4remotebackup.com’s service. it’s very inexpensive, and only requires you to forward a port on your site (getting clients to forward a port is sometimes problematic). GoTo Assist is another, as is remotesupport.com, and countless others. If you want to do this for free, MSN messenger has a built-in “Ask for Help” feature that you can use.
